Failures by a Tampa assisted living facility to follow its policies helped lead to an 81-year-old man’s death a state investigation found, according to the Tampa Bay Times.

MaynellScott, who had Alzheimer’s, walked out of Belvedere Commons on July 16 and was found three days later in a lake near Fletcher and North Armenia avenues, the Times reported.

It was not the first time he had wandered away and according to a report from the Agency for Health Care Administration, he should have been flagged as an escape risk and fitted with a GPS wristband, the Times reported.

The facility’s failure to follow that policy and others led to Scott’s drowning, the agency found.
